Connections to Primary Care — Healthcare for the Homeless
Metropolitan Family Health Network seeks to end the correlation between homelessness and poor health through our Health Care for the Homeless Program.
In this endeavor, we offer preventive health and mental care services, including physical exams, lab testing, immunizations and screenings. We also provide care for asthma, diabetes, flu, hypertension, exposure, malnutrition and emergencies, as well as referrals for substance abuse services and the full range of services provided by MFHN. There is also assistance in qualifying for Medicaid and NJ Family Care, and for housing and nutrition services.
Our program includes outreach to individuals and families on the streets and in shelters, plus interpretive/translation, transportation and case management services (assistance in obtaining social, emotional and financial help).
