The MFHN Diabetes Management Program

When you become part of our comprehensive Diabetes Management Program, we will assist you and your family to take a proactive approach to your diabetes so you can live a happier, healthier life.

A Team Approach
Our Diabetes Management team includes your personal doctor, a registered nurse, patient care technician, behavioral health specialist, patient care coordinator, and program manager. They will assess your individual needs and develop a personalized plan so you can self-manage your condition on a daily basis. Whether you’re diagnosed as pre-, type 1 or type 2 diabetes, the new skills and lifestyle modifications you’ll learn will help you live a satisfying life while reducing the risk of developing serious health complications such as heart disease, stroke, vision loss and nerve damage.

Innovative Diabetes Education
Our program includes one-on-one counseling, group sessions, community programs, and screenings. You’ll learn how diabetes develops and progresses, as well as how to monitor your glucose levels, adopt healthy eating and lifestyle habits, manage medications, and cope with chronic diseases, exercise, and foot, skin and dental care. They will also help you access healthcare and community resources.
